So I was fourteen, right? It was like 2 AM and I couldn’t sleep, so I was just flipping through channels. Landed on this documentary about some war somewhere, and there was this tent with a red flag and these three letters: MSF. The narrator said “Doctors Without Borders” and I remember thinking, who in their right mind becomes a doctor just to go get shot at? Like, my mom would kill me if I even thought about that. But anyway, that name stuck. Fast forward to now, and I actually had to research them for this essay. And man… I had no idea what I was getting into.
The Real Story Nobody Tells You
First thing I learned? MSF wasn’t started by some billionaire or government. It was literally a bunch of pissed-off French doctors. And journalists. Yeah, journalists were there from day one. This was 1971 in Paris, but the whole thing really started three years earlier in Nigeria during this war in Biafra. I’d never even heard of Biafra before this. Basically, the Nigerian military blocked off this region and people were starving. Like, actually starving to death.
So these young French doctors went there with the Red Cross to help. But the Red Cross had this rule—they don’t talk. They call it “silent diplomacy.” You see people getting massacred or starved, you keep your mouth shut because otherwise the government kicks you out and you can’t help anyone. One of the doctors, this stomach specialist named Bernard Kouchner, came back absolutely furious. He felt like by staying quiet, they were basically helping the bad guys. Like, if you watch a kid die of hunger and you don’t say who did it, what good are you really?
So they split. Thirteen of them—doctors and journalists—started their own thing. Their whole deal was that helping people and telling the truth about what you saw were the same job. They even made up this French word for it, témoignage, which is like… bearing witness, I guess? I don’t speak French, but you get the idea.
Their first mission wasn’t even a war. It was an earthquake in Nicaragua in 1972. Then a hurricane in Honduras. But the real test came in 1975 when all these Cambodians were running from the Khmer Rouge into Thailand. MSF set up their first refugee camp there, and everything fell apart. Supplies didn’t come. Doctors were stranded. The organization almost died right there. But they didn’t quit. They figured out they needed to be more professional, more organized. And they came back.
Getting Thrown Out of a Famine
Okay so everyone knows about the Ethiopia famine in ’84, right? “We Are the World,” Live Aid, all that. But here’s what I found that basically nobody knows. MSF got kicked out of Ethiopia. For telling the truth.
So they’re in this camp called Korem in northern Ethiopia. Tens of thousands of starving people everywhere. And MSF France starts noticing that the Ethiopian government is using the famine as cover to force people to move. Army trucks roll up in the middle of the night, take people to “resettlement” camps down south. Families torn apart. Kids wandering around in the freezing cold looking for parents who got taken.
There’s this MSF coordinator, Brigitte Vasset, and she talks about having to measure people’s arms to see who gets into the feeding center. Too big? Go away, no food for you. Small enough? Come in. She said she felt like an executioner. People dying right there in front of her, and the government won’t let them open more centers.
Then they find this “death house” in a village called Kelala. Naked women on the floor, covered in bugs and filth, can’t even move. The government took the healthier people and just… left these ones to die.
So MSF France goes public in October 1985. They blow the whistle on everything. And Ethiopia kicks them
out within days. Like, gone.
But here’s the crazy part. Almost everyone else stayed quiet. Bob Geldof—the Live Aid guy—actually sided against MSF. He thought they were being too political and would ruin the whole aid thing. Even MSF Belgium, who was also there, wouldn’t say anything. The French and Belgian teams stopped talking to each other. Can you imagine? Same organization, and they couldn’t even agree on whether to speak up.I read this and I just sat there. MSF got thrown out of a famine for doing what they thought was right. And yeah, the backlash made Ethiopia stop the forced relocations for a bit. But MSF lost access to the people they were feeding. So… did they win? I don’t even know. That’s the thing about them. There are no clean answers.
Doctors on Horses and Fighting TB
In 1979, when Russia invaded Afghanistan, MSF did this absolutely wild thing. They sent doctors across the border from Pakistan on horseback. Secretly. Into the mountains. Carrying medical supplies. Over ten years, more than 550 doctors and nurses did this rotation, setting up little hospitals in places where no one else would go. While being hunted. Like, actual movie stuff. But real.
And then there’s the TB thing. I didn’t know TB was still such a huge deal, but apparently drug-resistant TB is a nightmare. Two years of treatment. Ten thousand pills. Daily injections. Side effects that make you go deaf or literally lose your mind. And most of the drugs were fifty years old until pretty recently.
MSF runs these TB programs in Ukraine, Tajikistan, the Philippines. In Tajikistan, they have this pediatric program where kids who can’t swallow pills get their medicine mixed into syrup. They hire teachers so these kids don’t fall behind in school while they’re stuck in the hospital for months. They do group therapy because these kids are isolated from their families. But here’s the part that made me actually angry. Less than five percent of people who could benefit from new TB drugs are getting them. The drugs exist. They’re just too expensive or not available where people actually need them. MSF has been fighting for years to get diagnostic tests down to five bucks in poor countries. Five dollars. That’s it. And they still can’t get it done. They stood up at some conference in Bali and literally demanded it. That’s MSF for you. They’ll treat your wounds and then turn around and fight a pharmaceutical company.
The Nobel Prize Thing
In 1999, they won the Nobel Peace Prize. Cool, right? But what they did with the money is what got me. Dr. James Orbinski accepted it and in his speech just straight-up called out Russia for what they were doing in Chechnya. Right there on stage. But then instead of keeping the prize money for themselves, they used it to start this thing called DNDi—Drugs for Neglected Diseases Initiative. It’s a non-profit that makes medicine for diseases that don’t make money. Sleeping sickness. Chagas disease. Stuff that drug companies ignore because poor people get it and poor people can’t pay.
One of the founders said something that hit me hard: “Our patients are dying. Not because their diseases are incurable, but because as consumers they do not provide a viable market for pharmaceutical products.”
Think about that. MSF basically looked at the whole system and said, okay, if capitalism won’t save these people, we’ll do it ourselves. That’s not charity. That’s trying to rebuild a broken machine.
But They’re Not Perfect
I have to say this part too because my research showed me the other side. MSF messes up. They leave places when it gets too dangerous, and then what happens to the people there? In 2004, they pulled out of Afghanistan after five of their workers got murdered on some remote road. Taliban had been threatening them, the Afghan government didn’t care, and MSF said they couldn’t tell the difference between humanitarian work and military occupation anymore. So they left.
In 2024, things got really bad security-wise. Their clinics got raided, ambulances shot at, staff kidnapped. In Gaza, twelve of their own people have been killed since the war started. In Sudan, the fighting was so bad they couldn’t even reach people. They had to stop working in some areas, which meant local communities lost their only healthcare. That’s brutal.
And here’s the thing that keeps me up at night about them. They’re funded almost entirely by people like us—like 97.9% from private donations, not governments. That’s why they can say whatever they want. They can get kicked out of Ethiopia and not go broke. No government owns them. But it also means they’re constantly begging for money while trying to stay neutral. And neutral is hard. Really hard.
In Lebanon during their civil war, MSF treated soldiers from every side. Everyone respected them for it. But in Myanmar, working with Rohingya refugees, sometimes they speak up and sometimes they stay quiet to keep access. And people criticize them for both. There’s no winning. They publish these internal reports now where they admit when they got it wrong. How many organizations do that? But it also shows you that their job is basically impossible.
So What Do I Think Now?
That fourteen-year-old kid watching TV at 2 AM thought MSF doctors were crazy. Reckless. Now? I think they’re some of the most careful people alive. They do math I can’t even imagine. Walk into a famine knowing you might get expelled for telling the truth. Measure a starving person’s arm to decide who eats. Ride a horse into a war zone with antibiotics in your backpack. Fight for a five-dollar TB test while burying your friends.
“Doctors Without Borders” sounds like something from a movie. It’s not. It’s a promise that medicine doesn’t stop because someone drew a line on a map or because there’s no profit in it. And the people who keep that promise pay for it. With their safety, their sanity, sometimes their lives.I don’t think I could do it. Honestly, I’m pretty sure I’d cry on day one and want to go home. But I’m really, really glad they exist. Because right now, somewhere in a tent or a cave or a bombed-out building, there’s someone alive because a bunch of angry French doctors decided fifty years ago that keeping quiet wasn’t good enough anymore.
That’s the MSF I found. Messy. Stubborn. Flawed. And honestly? We need them more than ever.
